BUYER’S RIGHT TO CANCEL

If you wish to cancel this agreement, you may do so by emailing your request to info@futuremartialartsacademy.com within 7-business days of your first class. The notice must say that you no longer wish to be bound by this agreement, and that you would like to cancel your membership. This agreement may also be cancelled if Future Martial Arts LLC ceases operation.

ADDITIONAL COSTS

Uniforms, equipment, camps, private lessons, graduations, etc. are all additional costs and are not included in with tuition or registration. A uniform (GI), and new student kit (sparring gloves, boots, and headgear) is the required equipment for karate class. Gear must be purchased through Future Martial Arts. Please ask your instructor for gear requirement guidance.

By choosing a membership plan and signing above, I indicate that I have read and understand the terms of this Membership Agreement, and that I have read and signed a Liability Waiver Form. This agreement, and its information, will remain on file in a secure location, within Future Martial Arts LLC office unless the terms and conditions change. At that time, a new contract will be executed.

Acknowledgement Waiver

I understand that all sports carry a risk of injury and that Future Martial Arts is not responsible for injuries or loss suffered as the result of participating in these activities. From time to time Future Martial Arts will post photographs of our athletes, coaches, and supporters online. I will let Future Martial Arts know by email if I do not consent to my or my child’s photos being shared.

 Medical Authorization

In the event of an injury or medical emergency, I allow Future Martial Arts to provide first aid, arrange transport to a medical facility, and authorize emergency treatment as recommended by a doctor for the athlete named above.

I understand that all reasonable efforts will be made to contact the athlete’s parent and/or emergency contact if an injury or medical emergency occurs.
